It is a slur. There is no way to interpret the word without it being a slur. It’s an absolute equivalent to calling someone “mong” or “retard”.

Spastic is a medical term for the involuntary muscle spasms/stiffness as experienced by people with conditions like cerebral palsy. Using it in that context is not. Using it as a derogatory term for another person (or even yourself) is - it’s analogous to calling people/things/situations “gay” (when you mean “bad”).
