Given that Miku Miku Dance has had Kinect intergration for a while now, it is likely someone recording themself while playing but feeding it into MMD+Kinect. That’s how they get the motion yet not quite fluid lip sync.

MMD (Miku Miku Dance) has had Kinect intergation options for a long while now. Just skimming the video above, that looks to be what they are using most likely given the movement and lack of fluid lip sync.
