If you choose a custom Hawke when using a custom world state, you can alter his/her appearance. I chose to remove the facepaint.

It's not a bug. It's an interactive banter. Press R3 (or your controller's equivalent) to open the wheel when near the NPC talking to you.

Yes, because the interact symbol will disappear when you press X (PS4). It happens often with NPCs.
