I wouldn’t go so far as to call DA:I’s characters sex dummies. I thought most of them were pretty well-written (or at least memorable); especially Bull, Sera and Blackwall.

No, only the first missile in a salvo has a chance for a head hit, the others automatically go somewhere else (if the first one hits.)
All mechs actually have the same head armor and structure. Which means enemies with AC/20s are always a priority target, as that’s the only weapon that can actually take out your cockpit in 1 hit (standard PPCs don’t do enough damage.)
I’ve also lost a 10/6/9/9 guy to a kick to the head. The mission was so incredibly hard (and the salvage so good), that I didn’t savescum him back.

According to a reddit AMA with one of the designers, it’s not a bug, but a feature wrong in-game description. It’s supposed to mean your intel was wrong and you’re outnumbered, and it’ll get fixed next patch.
