Don’t be naive; do you really think the publisher cares about your feelings? Offense could be construed at LITERALLY anything, depending on an individuals sensibilities. No company would ever release anything if “someone’s feelings might get hurt” was a legitimate concern.

Many times, ESRB submission is a collaborative process, they know what kind of ratings developers hope for when they submit their games, so if they see that some content will bump a game to a higher rating, they will definitely tell them to remove that content if they want to…
Not arguing with what you say, but ESRB doesn’t censor games. They issue ratings adapted to the game content. For customers, they’re just indications. Developers are free to adapt their game to get a lower rating (which can impact retail and marketing) or to go with what they have.

Am I? I certainly exaggerated the actual tweet but she said she was glad he was gone. Being happy (glad) that someone is dead isn’t particularly uncommon but announcing such a thing publicly comes across as very very cold and certainly unprofessional.
Please, by all means find the offending tweet from the twitter spat written by the person she was insulting that adds the context that I apparently missed...

Taking the original twitter spat alone, her behavior was ridiculous. She posted a multi-tweet essay, essentially, to a public space. Someone respectfully commented on that essay and she began passively insulting him.
