4% of people finished the game on Insanity? That's really sad. ME3 had the easiest Insanity run because the gameplay had improved so much since the crappy ME where on Insanity, you couldn't damage the enemies and they couldn't damage you, and the horrid ME2 where if you popped out of cover for a second you were dead.

Amazing game, I got it on sale during the Christmas sale on Xbox Live, well worth the full price though and I'll definitely buy it for the PC.
