Sending an input stream to a device to be displayed has been around for a lot longer than 20 years. It is just done in a very roundabout fashion in this case.

It doesn't "stream" anything (as in create a digital video stream to be watched over twitch). All it does is display the input "stream" of letters fed to it by a twitch chat, which is then captured by a traditional capture device and put back on twitch.

i cant remember when Moonfang was introduced, but at least last expansion, during the Mists of Pandaria pre-patch (I.e like now, just with players being LV 85 and the wolf being 90) we had the same situation.

You'll need access to the keyboard while you play (which is why pad is only ok). There are too many functions for targeting ships, wingman commands, energy management, weapon selection, ect for it to be played just with a gamepad.
