It might be simple misclick. There may be a folder with a bunch of graphic assets that are stored for flags, and rather than searching, the person popped into the folder and grabbed the wrong asset.
I mean, another solution is to be less technologically ambitious. You can make a great, well designed game with excellent mechanics and fun game play, that has deliberately extremely limited or minimal graphical requirements.
