Bar codes can be tracked as well. Not as deeply as from an app, but it can Identify geography and demographics. And since your printing it out, it can be tied to IP, which is geography and company based. They can see this particular clinic is printing out this coupon, which was redeemed at these particular store(s).

Cc companies never lose money. They charge the merchant to run the transaction and they charge the card holder interest if they don't pay the balance. If a customer reports fraud, then the merchant is held for the loss.

Sounds like your using a debit card, which has a different set of rules.
