9 months salary vs 12 month salaray depends upon the district. Most of the ones around me spread the salary out over 12 months, including the 3 districts I have worked for (2 in Missouri, 1 in Illinois).

Like 95% sure. I get the TDAP regularly every 10 years (just a month ago, in fact). I am older (born 1973), and I don’t know if 2 doses were recommended back then. I had one for sure as a kid, but then my undergrad in 1991 had me get another, and then both grad schools in 1999/2010 had me do it again.
