My 10 month old had emergency surgery (for an intussusception) on a Saturday that saved his life. Insurance company said it should have been preauthorized so they wouldn’t pay it in full. He would have been dead on Sunday morning. (Which I imagine might have saved BC/BS money)

Me too, and then before my husband changed jobs, his insurance started including the 3D imaging. Now, for a mammo I have to go to a provider I HATE that made me come back for an ultrasound even though the doc had prescribed it, and the insurance covered it.
