Well, it seems like a combination of backward facing and distraction. In my case the kids were always forward facing in the back seat, but it was usually a straight shot from home to sitter and no cell phones back then. Plus, a nearby commute. I wonder how a long commute might factor into all this?

It's a tragedy all around although I still can't figure out how someone leaves their kid in the car all day. How is it possible to forget your kid? My kids are grown, but I was a working parent for decades and I can't figure out how this happens. :\
