With regular diving, if something goes wrong, you always have the option to surface (granted, depending on depth, you have to be cautious in doing that).    In cave diving, you may find yourself in a situation where you can’t surface.

In short, all of the above.

Pretty much everything you listed. Cave diving is super dangerous. It’s easy to get turned around in the dark, not even knowing which way is up, or get stuck in a squeeze and run out of air.
