The person in that car most likely drove that stretch of road every day of the week for years. Years of putting up with getting cut off, bumper to bumper tie ups, road construction, asshats getting behind them and honking even when there is clearly an unmoving line of cars in front of them.
That is extra armor in case one of the lisence plates could in any way be misinterpreted to mean something that was even mildly insulting to italians.
